A case of Hashimoto¡¯s encephalopathy with focal cerebral perfusion defect

Abstract

|
Hashimoto encephalopathy (HE) is a rare clinical condition associated with Hashimoto thyroiditis. Patients with HE usually have three specific characteristics: (1) elevated thyroid autoantibody titer, (2) neuropsychiatric symptoms such as seizure, altered consciousness, cognitive dysfunction, or speech and behavior impairment, and (3) good response to steroid therapy. Although the etiology and causative mechanism of HE are still unknown and precise methods for diagnosing HE have not been established, current diagnostic strategy depends on the findings that (1) the protein content of the cerebrospinal fluid is elevated, (2) brain waves become slower on electroencephalogram, (3) subcortical signal intensity increases on brain magnetic resonance imaging, and (4) cerebral perfusion verified by using single photon emission computed tomography (SPECT) decreases through the entire brain. We report a case of typical HE, showing a focal decrease in cerebral perfusion as verified by SPECT, along with a literature review. (Korean J Med 74:S222-S227, 2008)
